Our culturally responsive Case Management addresses your ongoing needs, supports you post crisis, and provides essential support within social housing.
Case Management
A Mental Health Worker will work closely with you to understand your unique needs. The goal is to support you and your family through the recovery journey using clear strategies for goal-setting, planning, and reflection.
Mental Health Workers conduct assessments, create care and safety plans with realistic goals, and provide supportive counselling to you and your family. They also strengthen connections with community resources to ensure you will have the support you need at every step of recovery.
Case Management provides:
- Assessment and care planning
- Information and referral
- Service coordination and care conference
- Symptom management
- Education and skill building
- Supports for daily living
- Supportive counselling
- Linguistic and cultural interpretation support
- Escort for medical and legal appointments
- Advocacy
Case Management is free for everyone. To help reduce wait times, we encourage those with other service options to explore them first if available.
Our services are available to individuals who:
- Are 16 years or older and experiencing mental health challenges or a mental illness
- Live in the City of Toronto (For those outside Toronto with no local services, we may consider requests if you can come to Toronto for support)
- Voluntarily consent to receive services
We accept referrals from individuals, families, friends, and health or social service providers.

Post-Crisis Short-Term Case Management
Post-Crisis Short-Term Case Management
In partnership with the Canadian Mental Health Association, Toronto and Toronto Community Crisis Service (TCCS), this service supports Cantonese, Korean, Mandarin and Vietnamese speaking individuals in North York who have recently reached out to TCCS during a crisis and need follow-up help. Our Mental Health Workers assist clients in understanding what led to the crisis, connecting with the right resources, recognizing their strengths, and building skills to manage challenging situations effectively.
Services include:
- Conducting assessments
- Creating safety plans
- Setting achievable goals with a 3–6 month timeline
Call 211 to be connected to the TCCS.
Supportive Housing
Individuals and their families are supported to access affordable, stable housing and a safe living environment while working toward personal recovery goals. Clients live independently in the community—either alone or with family. HF Care provides the apartment at a subsidized rate and Case Management to support recovery. Case Management Mental Health Workers help clients navigate the health care system and connect with social supports that promote well-being.
Mental Health Workers provide education and skill-building in areas such as:
- Solution-focused problem solving
- Risk management
- Coping with emotions and mental health challenges
- Accessing community resources
- Building positive relationships and life skills
Who Can Access Our Services:
- 16 years or older with mental health challenges
- Ready and able to live independently
- With income (e.g., earned income, Ontario Works, Ontario Disability Support Program, Old Age Security)
- With legal status in Canada (Permanent Resident or Canadian Citizen)
- Willing to accept Case Management from HF Care or other mental health agencies
- Face linguistic or cultural barriers to accessing other mental health services

Asian Community Psychiatric Clinic (Partnership)
Psychiatric assessment and treatment in a culturally safe, language-specific setting. Through our partnership clinic, individuals and families can access bicultural psychiatrists who understand the lived experiences of Asian communities.

Recovery Programs
Through education, community, and lived experience, our peer-led recovery programs empower individuals to build connection and purpose.

Family Support
Support for families navigating a loved one’s mental health journey. We provide education and culturally responsive counselling to help families strengthen relationships together.

Mental Health Education
Community-based mental health education that reduces stigma and builds skills. Our workshops and outreach programs are culturally tailored to meet the needs of our diverse Asian communities.

Youth and Family Services
Culturally responsive support for youth and families facing emotional, behavioural, or mental health challenges. We create safe spaces where young people can build confidence and connection.

National Professional Trainings
Training for service providers seeking culturally responsive approaches to mental health care. Our programs strengthen cultural competency, improve engagement, and equip professionals to better support diverse communities.
